Senior Living Transitions

When the time comes for your parent or loved one to make the transition back home or into a long term care facility, Covenant Senior Resources is here for you. We know first-hand the psychological and physical strain that changes in a living environment or care facility can have on older clients and their care giver families. You can rest assured that we will make the “New Adventure” as comfortable and as effortless as possible. Services provided include:

A complete psychosocial, functional and clinical assessment of the elder to discover any health or safety issues related to moving
A detailed and individualized care and wellness plan
Coordinating all aspects of the move
Linking you with a mortgage broker and oversee the selling of the home
Sprucing up the home to give it more curb appeal, thereby raising its market value (painting, necessary lawn and garden maintenance, household cleaning, etc)
Hiring an effective and trustworthy appraiser / estate sale planner to oversee the selling of household items and valuables
Linking you with a respected senior long-term care planner to advise on how to invest the proceeds from the sale of the home to effectively pay for medical and living expenses in the future
Linking with a satisfactory storage facility, if needed
Disconnecting all utilities
Submitting the necessary changes of address

The best part is that you and your parent will have a dedicated care manager to guide you every step of the way. Your care manager will be available 24 hours a day / 7 days a week and will act as your liaison and your loved one’s staunch advocate, overseeing the transition and quickly responding should problems arise. We only utilize licensed and bonded service providers and our support staff is available anytime to answer questions and address concerns. So let us worry about the extra stuff, so you can spend your time where you’re needed most…
