Advance Directive--a term used to describe two kinds of legal documents, living wills and medical Powers of Attorney. Advance Directives documents allow a person to give detailed instructions about theri future medical care should she/he be deemed unable to participate in medical decisions relating to themselves due to serious illness or incapacity. Each state regulates the use of advance directives according to different legislation.

A geriatric care manager is a professional who assesses an elder's ability to live independently in a home environment, develops an appropriate care plan for services and equipment, and organizes needed home services. This person may monitor and augment services on an ongoing basis, or trouble shoot as particular problems arise."
