A geriatric care manager is a professional who assesses an elder's ability to live independently in a home environment, develops an appropriate care plan for services and equipment, and organizes needed home services. This person may monitor and augment services on an ongoing basis, or trouble shoot as particular problems arise." (Bookman et al., 2007) “Geriatric Care Managers specifically trained in geriatric care management, and provide case management services on a fee-for-service basis to individual clients" (DHHS).
